As parent / guardian,

I confirm that:

• Junior named has completed a continuous open water swim of at least t3/4 of the length of this swim within 30 days of the event, in water of the a similar temperature
• Junior named understands the safety and rescue procedures and their own limitations
• Junior named  is fit and has no medical issues that will affect his/ her ability to complete the event.
• Junior named is aware that the race organisers will check his/ her progress regularly throughout the race and have the discretion to instruct him / her to withdraw from the race.  Any such instruction must be followed.
• Junior named  is aware of the race route, its length, the depth and temperature of the water, the direction of significant currents and any other hazards.
• Junior named  is aware that a mass start carries intrinsic difficulties, which can be frightening and present risks, which could include pushing and kicking.
• Junior named is aware that providing a signature at registration confirms knowledge of and acceptance of the swim proficiency declaration.

In entering on behalf of a junior I confirm I have the authority to act as this junior’s guardian and that I agree with all of the above statements on their behalf.  I understand that the junior is entering the water at my discretion and that it is my responsibility to ensure the junior is able to manage the swim and water conditions.  I am aware of the risks of open water swimming and that, like all sports it carries the risk of injury, paralysis or death. I am entering this junior as a parent or guardian  to this event of my own free will. I hereby agree to keep ICS and its officials, rescue crews and all sponsors free from any loss or legal claim however caused by my participation in the event.
